USA - NTSB Publication - Safer Seas 2013

The United States National Transportation Safety Board (NTSB) Office of Marine Safety has recently published Safer Seas 2013: Lessons Learned From Marine Accident Investigations, based on a collection of investigation reports that were released last year.

Each investigation has been summarised and the probable cause has been identified. Links to the full investigation reports are also provided.

Safer Seas 2013 may be used as a learning tool by both ships’ personnel and shore management, raising awareness of the errors, acts, failures and / or omissions that were instrumental in causing shipboard accidents.
